Sensitive categories
The sensitivity classifier is the core of this feature: every incoming condition, medication and document is classified at the ingestion door, every downstream gate depends on its output — and because a missed Part 2 record is the most consequential error this platform could make, the classifier's uncertainty is managed in the open: low-confidence classifications queue for human review below, uncertain stays withheld (fail closed), and every reclassification is audited.

Ask the assistant about a patient who has withheld records: it reports the counts and refuses the content — the honest posture the platform takes, demonstrated on data you can inspect.
Classifier review queue — low-confidence classifications
Documents whose category rests on thin evidence — a single term in a long note. Fail-closed means each stays withheld under its category until a human decides; confirming or clearing is an audited decision (segmentation.reclassified).
