Patient instructions

The model writes what the patient takes home — plain language, 6th-8th grade reading level, drawn only from the chart in context. Then a second pass turns on its own draft: the no-new-assertions check verifies every clinical claim against the record and fails the draft on a single invented instruction. The check is a release gate, not a report — a failed draft cannot reach the signature queue at all.
