For every dollar a hospital is paid, it billed $5.03.

A hospital bill starts from a list price almost nobody pays in full — insurers negotiate it down, Medicare sets its own rate. The person who pays closest to the list price is the one with no insurer to negotiate for them. That gap is where medical debt begins.

Project Crossfoot puts the public prices in one place: what every hospital billed Medicare and what it was paid, for every inpatient admission and outpatient service; what a doctor's visit costs in every state; and what 2,673 hospitals' own price-transparency files say they charge. Each file has been checked against itself. 342 of the 2,673 readable ones contradict themselves.

What is in this dataset

Every public price the US healthcare system publishes about itself, in one database: what every hospital billed Medicare and what it was paid, service by service; what clinicians charge in every state; what a 50-state sample of hospitals' own mandated price files list; and the county-level debt and health outcomes those prices sit beside. Every tile below is a door — click one for the rows behind the number.

02 · Findings

Hospital price files, checked against themselves

Federal rule 45 CFR 180 requires every hospital to publish its standard charges in a machine-readable file, and since 2024 to point to it from a cms-hpt.txt at its web root. Nobody checks whether the numbers inside agree with each other. 3,109 hospitals across every state were sampled and 2,673 of them read in full — every CPT, HCPCS and MS-DRG line — and compared on a fixed basket of 49 services. Each check is one way a file can disagree with itself: a cash price above the list price it discounts, a negotiated rate outside the minimum and maximum the same file states, a file older than the rule's annual refresh. No finding is a judgment call — every one is re-derivable from two figures in the same published file.

Across the 2,673 readable files the checks produced 7,735,443 findings: 28,011 hard contradictions, 45,477 warnings, and 7,661,955 notes recording what a file did not disclose.

Where the system runs hottest

Two maps, one question each. The first: for every dollar a state's hospitals are actually paid, how much did they bill — across every admission type, weighted by discharges. The second: what share of the state's adults carry a medical bill that went to collections. The maps are computed from different sources — Medicare's files and a panel of credit records — and where they run hot together, that is worth a long look.

One procedure, every state

A fixed basket of 49 services that mean the same thing everywhere — the visits, labs, scans, procedures and admissions most people will meet. For each: what hospitals billed Medicare on average, what was actually paid, and where the sample has it, the hospitals' own listed and cash prices. Every other code is in the dictionary, with a verdict on whether it is a code at all.
